Integrative Health and Wellness Assessment ™
Holistic nursing is an expression of nursing practice that aims to heal the whole person, honors relationship centered care and recognizes the connectedness of self, others and nature. Holistic nursing is a recognized specialty by the American Nurses Association and serves as the basis of my advanced nursing practice. As a holistic nurse scientist I am motivated by my strong faith in God, the spirit, family and belonging and a close connection with nature. These attributes come forth in everything I do as a scientist and healer.
This integrative health and wellness assessment (IHWA) was designed by holistic nurses Dr.s’ Barbara Dossey, Susan Luck and Bonney Gulino Schaub 1 to help you become aware of your human potential. Healing is more likely to occur when you attend to your life balance and satisfaction, which is the foundation of the IHWA model. This short survey will evaluate your wellness in the following categories: Life balance & satisfaction; Relationships; Spiritual; Mental; Emotional; Physical; Environmental; and Health Responsibility. Take your time and answer from your heart. This Integrative Health and Wellness Assessment (short form) is intended for informational purposes only. It is not a substitute for professional medical advice, diagnosis or treatment.
